If your team is part of a Domestic Club do NOT complete the online form below. Contact your Club directly to lodge your complaint and the club will lodge the complaint on your behalf with the association if required.

- The registered Coach or registered Team Manager ONLY to complete this form on behalf of the team;
- Concerns regarding the conduct of a match should be raised with the Referee Supervisor on match day via the registered Team Manager or registered Coach only.
- Match protests must be submitted within 48 hours of the completion of the match.
- If the feedback or complaint is NOT in regards to a matter directly associated with a team or club, is serious of nature or highly confidential then the matter should be forwarded directly to manager@morningtonbasketball.com.au or if the matter is urgent call 0427 305 536.
- You must provide true, accurate and detailed information.
- This form is not to be used for grading complaints refer to MDBA By Law 14.
- Note that scoring discrepancies are not reviewed. Match results are not changed once the match has been signed off by the referees. Scoring discrepancies must be reported to the Referee Supervisor at the time.
