Any member of the Junior Breakers program wishing to lodge a formal complaint must follow the below process:
- If you suspect a child is in immediate danger or at risk of harm, call 000 immediately and complete the MDBA Child Safe Incident Form.
- All Incidents, Concerns, and Feedback must be submitted by the registered Team Manager using the link provided on this page.
- In the first instance, complaints should be raised directly with the registered Team Manager. If required, the Team Manager will then submit a formal complaint using the link on this page.
- If it is not appropriate to raise the matter with the registered Team Manager due to a conflict of interest or similar concern, a parent or guardian may lodge a complaint directly using the link on this page.
- MDBA will acknowledge and respond to complaints within seven (7) business days of lodgement.
- MDBA will determine whether the matter can be resolved internally or whether it should be referred to the Basketball Victoria Tribunal or the Basketball Victoria Integrity Unit.
