INJURY REPORTS

Injury Report Form

Any member or visitor that injures themselves whilst in attendnace at a venue being operated by Mornington District Basketball Association must:

  1. Report the matter to the staff member in charge of the venue
  2. If at an external venue (i.e training venue) report the matter to the venue
  3. Complete the online Injury Report Form above.

All teams must have the Head Coach App downloaded and use this as a tool in the event of a suspected concussion. The above Injury Report Form should also be completed.

Basketball Victoria provides a level of insurance to all registered members of Affiliated Associations.

Basketball Victoria’s insurance provider is V-Insurance Group.

Mornington Basketball does not administer the insurance process or provide advice on insurance related matters. A registered member who sustains an injury, should visit the Basketball Victoria website and follow the steps to submit your insurance claim. Once the claim form is complete, please contact the Mornington Basketball General Manager to make a time to have the Claim Form approved and signed – manager@morningtonbasketball.com.au

All coaches and team managers should be aware of any players who suffer from Anaphylaxis, Asthma, Epilepsy, Diabetes and any other conditions which may require emergency treatment, and understand how to recognise the signs of a problem.

Parents are encouraged to inform coaches and Team Managers if their child has a medical condition, however please remember that it is not mandatory for team officials to have first-aid training.

Basic First Aid kits are available at each competition venue.
